You can help improve the lives of individuals in Georgia with mental illnesses, substance use disorders, and intellectual/developmental disabilities simply by completing a brief, anonymous questionnaire.
To participate, simply click on the link below:
This questionnaire is part of a survey being conducted by the Regional Advisory Councils of the Georgia Department of Behavioral Health and Developmental Disabilities (DBHDD). Your anonymous responses will be used to identify and recommend community/area priorities for improving DBHDD disability services locally, regionally, and statewide.
All Georgia residents are invited to participate in this survey. Please invite your friends, family, and colleagues to share their opinions. Speaking out about unmet needs and/or quality of services will go a long way toward improving the lives of citizens with disabilities across Georgia.
This questionnaire is also available in Spanish. The link to the Spanish version is www.surveymonkey.com/r/spGeorgiaNeeds.
If you have any questions, please contact our DBHDD community involvement intern at email@example.com.
Thank you for your participation.
